Obverse: Portrait of the Jewish pharmacist Jonathan Pereira (1804-1853) with the inscription "Jonathan Pereira MD FRS FLS". Reverse: A wreath, in its center the inscription "Materia Medica. Awarded by the Pharmaceutical Society of Great Britain, Prize Medal Founded 1854". Name of receiver and the year are engraved on the rim: "John Thomas 1887".
